What earns a record
A retained producer record must identify an intervention, evidence window and observed or explicitly unmeasured outcome. Zero trials is not evidence of success.
Illustrative example · not a recorded event
A trial changes one parameter, states an expected effect, and later compares the observed window. Without a control, even improvement remains directional evidence rather than proof of cause.
